Root cause analysis

Working out why a call went wrong, rather than only recording that it did.

A failed call has a proximate cause (the agent did not offer the callback) and an underlying one (the callback tool was down, or nobody had told them the policy changed). Coaching the first without fixing the second produces frustration and no improvement.
